More capacity for the patients who need it most

Youwell gives the public sector a more sustainable model for patient follow-up - more patients can be followed up with the same resources, whether in hospitals, municipalities, across levels of care, or in national services.

Digital patient pathways deliver more structured follow-up and better use of resources

Challenges in public healthcare

The need for follow-up over time is growing faster than the availability of staff, and much of the work is still tied to the physical consultation room.

  • More patients with long-term conditions, without a matching growth in staffing
  • Limited capacity to follow up between consultations and after discharge
  • Assessments on paper or in separate form tools that have to be re-entered
  • Pathways split between hospital, municipality, and GP, each with different systems
  • One solution per indication, resulting in many small systems to manage
  • Little structured data for quality work, management, and research

Digital pathways don't solve the capacity challenge alone, but move part of the follow-up to where it can happen without in-person visits, and make it more predictable for both patient and clinician.

How Youwell creates value

Youwell is a digitalisation tool for the public sector - built for authoring, delivering, and analysing digital patient pathways, which the organisation itself owns and operates.

  • One platform for multiple pathways - the same tool for mental health, somatic care, and rehabilitation
  • Your own clinical staff build and change content, forms, and pathway logic themselves, without a development project
  • Access to a library of validated programmes that can be used directly, instead of being built from scratch
  • Assessment, content, tasks, logging, and dialogue in the same patient interface
  • Clinician overview with progress, flags at defined thresholds, and unread messages
  • Stepped offerings: open knowledge modules, unguided self-help, and guided pathways in the same solution
  • Patient-reported data gathered in a structured way, with exports for quality work and research
  • Roles and access adapted to hospital and municipal structures, with an access log
Use cases

Typical use cases

Below are the use cases we see most often - they can be combined and adapted, and are all built from the same building blocks: modules, forms, diaries, logging, tasks, dialogue, video, messages, notifications, flagging, and data exports.

Guided internet-based treatment

The clinician assigns the patient a structured programme with modules, tasks, and exercises, and provides written guidance between modules. Progress and form responses appear in the clinician's overview and form the basis for the next piece of feedback. The clinician follows several patients in parallel without each follow-up requiring a scheduled appointment.

Self-help and coping programmes

Patients and citizens can get access to structured self-help and coping programmes without ongoing clinician follow-up - a low threshold, no waiting time, and capacity freed up for those who need closer follow-up.

Digital home follow-up for long-term conditions

The patient logs symptoms, measurements, and medication use at home according to an agreed plan. The logs are gathered into one view for the follow-up service, and defined thresholds trigger flags so that deviations are seen. Contact happens by message or video when the logs call for it, rather than at fixed intervals.

Assessment before the first consultation

The patient fills in assessment forms digitally in the period between referral and appointment. The clinician has the answers available at the start of the consultation, and the same forms can be repeated at defined points later in the pathway. The forms can be set up by the clinical team themselves, both validated instruments and their own.

Follow-up after a stay, procedure, or discharge

After a stay or treatment, the patient gets a plan with content, a training programme, reflection tasks, and reminders, often combined with scheduled video calls. The treatment provider sees which parts of the plan have been completed, and can adjust it along the way. This maintains contact during the period when the need for support is often greatest.

Stepped services in the municipality

Municipal services can offer the same platform across several levels, from unguided self-help to a guided programme with an assigned contact person. Citizens can move between levels without switching tools, and the service retains an overview of who is at which level.

Structure and alerts in patient pathways

Activities, forms, and touchpoints in a pathway are set up as a timeline with alerts to the patient and to the coordinator. The overview shows where each patient is in the pathway, and which logs are missing. Inactivity beyond a defined number of days can be flagged automatically.

Collaboration between hospital and municipality

The same pathway can involve staff from several organisations, with roles and access set per patient. External resources are only given access to the patients they're registered on, and all access is logged. The patient doesn't have to deal with one solution per level of care.

Patient-reported data in quality work

The organisation sends PROM and PREM forms to defined patient groups at fixed times, tagged for later export. Aggregated responses are exported to the organisation's own analysis tools and used for quality improvement, reporting, and professional development. Pathway reports show how many have received and completed each form.

Platform capabilities

Author, deliver, and analyse - in one platform

Author

The clinical team sets up pathways, content, assessment forms, and flagging rules - with version history and approval before publishing.

Deliver

Clinicians assign pathways, follow up logs, dialogue, appointments, and video, and handle events and flags from one overview.

Analyse

Data from the pathways is used for quality improvement, management information, and research, with exports in CSV or Excel with a codebook.
